Causal Factor Analysis: More Useful Than RCA?

When organizations conduct safety investigations of hazards or incidents, their purpose should be to prevent the accident from happening again. Often, Root Cause Analysis (RCA) falls short of this goal because it may ignore more “actionable” causal factors. Join us to learn the Causal Factor Analysis technique, including how easy it is for people to read and understand a report that uses this format. Attendees will receive two templates for building their own future causal factor statements.
